The attached Purchase Order having been approved, NOTICE is hereby given to PESOPLUS DRUGSTORE to proceed on the Supply and Delivery of Medical and Laboratory Supplies for President Ramon Magsaysay Memorial Hospital (PRMMH), this Province, within Twenty-One (21) Calendar Days upon receipt of this notice.
Upon Receipt of this NOTICE, you are responsible for performing the services under the terms and conditions of the Agreement.
Please acknowledge receipt and acceptance of this N O T I C E by signing both copies in the space provided for below. Keep one copy and return the other to the Bids and Awards Committee, this province.
