The attached Purchase Order having been approved, NOTICE is hereby given to LOURD PHARMACY to proceed on the Supply and Delivery of Lot 1: Drugs and Medicines for President Ramon Magsavsay Memorial Hospital (PRMMH) and San Marcelino District Hospital (SMDH), this Province, within Twenty-one (21) Calendar Days upon receipt of this notice.
Upon Receipt of this NOTICE, you are responsible for performing the services under the terms and conditions of the Agreement.
