The attached Purchase Order having been approved, NOTICE is hereby given to LOURD PHARMACY to proceed on the Supply and Delivery of: Lot 1 Drugs and Medicines for Ospital ng Sta. Cruz (OSC), Candelaria District Hospital (CDH), President Ramon Magsaysay Memorial Hospital (PRMMH) and San Marcelino District Hospital (SMDH), this Province, within Twenty – One (21) calendar days upon receipt of this notice.

Upon Receipt of this N O T I C E, you are responsible for performing the services under the terms and conditions of the Agreement.

Please acknowledge receipt and acceptance of this N O T I C E by signing both copies in the space provided for below. Keep one copy and return the other to the Bids and Awards Committee, this province.
