This is to notify you that your Bid dated October 27, 2022 for execution of the Repair Works at Jail Facility, Zambales Provincial Jail; Construction of Multi-Purpose Building and Concreting of Concrete Pathways, Brav, Lipav-Dinain-Panibuatan; all in Iba, for the Contract Price of equivalent to Seven Million Four Hundred Forty Four Thousand Three Hundred Seventy Eight and 21/100 Pesos (P7,444,378.211) is hereby accepted.
You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders. Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.
Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:
1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’S approved Construction Safety and Health Program
