This is to notify you that your Bid dated October 11, 2022 for execution of the Renovation of health care-related facility in the provision of quality TVET System, TESDA-ZAMBALES, for the Contract Price of equivalent to Three Million Four Hundred Sixty Four Thousand Five Hundred Twenty Six and 43/100 Pesos P3,464,526.43) is hereby accepted.

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders. Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
