Dear Ms. Cabojal:

This is to notify you that your Bid dated January 19, 2023 for execution of the Construction of Two (2) Storey Multi-Purpose Building as Document Storage Facility for the Commission on Audit (COA) and the Provincial Government of Zambales (PGZ), PSAO Compound, Palanginan, Iba, for the Contract Price of equivalent to Eleven Million Nine Hundred One Thousand Two Hundred Twenty Seven and 51/100 Pesos (P11,901,227.51) is hereby accepted.

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders. Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
