Dear Mr. Saldania:

This is to notify you that your Bid dated February 2, 2023 for execution of the Construction of Ten (10) Classroom 2 Storey E-Building for Canaynayan Elementary School, Sta. Cruz, Zambales, for the Contract Price of equivalent to Twenty Six Million Eight Hundred Seventy Eight Thousand Nine Hundred Seventy One and 16/100 Pesos (P26,878,971.16) is hereby accepted.

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders. Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
