This is to notify you that your Bid dated December 21, 2021 for execution of the Concreting of Farm to Market Road (Hamon, Baquero and Sto. Niño) and Purok 2 and 3, Brgy. Taposo; Construction of Drainage Canal along Brgy. Road, Brgy. Pamibian and Brgy. Babancal, Candelaria; Concreting of Brgy. Road (San Roque Street), Brgy. Yamot; all in Candelaria, for the Contract Price of equivalent to Eighteen Million Three Hundred Twenty Four Thousand Nine Hundred Ninety Nine and 74/100 Pesos (P 18,324,999.74) is hereby accepted.

 

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders.  Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

 

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

 

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
