This is to notify you that your Bid dated December 16, 2021 for execution of the Construction of Drainage Canals, Brgy. Naculcol and Brgy. Palis; Concreting of Brgy. Road including the construction of Drainage Canal, Parel; Concreting of Brgy. Road at Purok 6, Brgy. Paudpod, at Purok 6 (Rosaban), Brgy. Poon-Bato, Santiago, and Tampo; all in Botolan, for the Contract Price of equivalent to Eight Million One Hundred Seventeen Thousand Two Hundred Forty Five and 34/100 Pesos (P 8,117,245.34) is hereby accepted.

 

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders.  Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

 

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

 

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
