This is to notify you that your Bid dated December 23, 2021 for execution of the Concreting of Caniyan Farm to Market Road, Brgy. East Burgos; Construction of Additional Slope Protection Works along Farm to Market Road, Brgy. San Isidro; Concreting of (Barangay Road), Angel St., Sitio Sto. Niño, Brgy. Rabanes and Brgy. Buhawen; all in San Marcelino, for the Contract Price of equivalent to Ten Million Nine Hundred Four Thousand Nine Hundred Ninety and 96/100 Pesos   (P 10,904,990.96) is hereby accepted.

 

You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated in the Instructions to Bidders.  Failure to provide the performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security.

 

Likewise, you are required within the stipulated period to submit to us this Notice with your signed “conforme” thereon, with the following supplemental documents, to wit:

 

1. Construction Schedule and S-Curve
2. PERT/CPM Diagram
3. Manpower Schedule
4. Equipment Utilization Schedule
5. Construction Schedule
6. DOLE’s approved Construction Safety and Health Program
